55/F Type 2 Diabetic since 11 years





 A 55 year old woman a housewife and resident of west bengal belonging to upper middle class according to modified BG prasad classification presented to the OPD with chief complaints of 

-fatigue since 2 years 

History of present illness

patient was apparently asymptomatic 11 years ago , when she had episodes of insomnia and fatigue , for which she visited the hospital and was incidentally diagnosed with type 2 diabetes mellitus , she was adviced lifestyle modification i.e; diet and exercise , in 2014 she presented with similar complaints and was advised medication. Patient has been regularly taking medication since 9 years 

h/o polyphagia

no history of polydipsia, polyuria,nocturia 

n/h/o SOB






history of past illness 

known case of hypertension since 23 years , uses medication regularly 

n/k/c/o tb , asthma , epilepsy 

treatment history 

-diabetes : metformin and glimepiride

-hypertension : amlodipine

family history 

mother is k/c/o hypertension since 56 years

menstrual history 

menarch : 13 years

regular periods ; 4/30

not associated with pain 

not associated with clots 

personal history 

diet : mixed 

appetite : normal

exercise : walking 1-2 km each day 

addictions : tobacco containing paan : 5-6 paan each day since 2005 

no known allergies

general examination 

patient was c/c/c and oriented to space and time

 A 65 year old Hindu woman a housewife a resident of west bengal having a Bsc degree and belonging to middle class according to modified kuppuswamy scale presented to the general medicine OPD with  chief complaints of  -constipation since the past 3 years  -continuous dribbling of urine since the past 4 years    History of presenting illness   Patient was apparently asymptomatic 26 years ago , when she developed continuous dull aching pain in both the knee joints , which was exaggerated in the mornings and was bearable throughout the day , it was aggravated on walking and relieved on rest  associated with redness around the knees no history of trauma no history of evening rise of temperature  no history of cosmetic deformity(varicosity, skin discoloration) -she underwent a bilateral knee replacement for knee stiffness  -is on DMARDS since 25 years  patient was incidentally diagnosed with hypertension 20 years ago during a follow up visit  -she was started on antihypertensive medicat
