On going case

 A 60 year old woman who is a coolie by profession and a resident of Nalgonda had come to the OPD with the following complaints 

-Dizziness since 2 days 

-Headache since 2 days 

History of present illness 

Patient was apparently asymptomatic ,she reported feeling  dizzy since yesterday afternoon , she had her lunch and reported feeling dizzy on sitting and standing as well and it subsided with rest 

She also had headache associated with her dizziness 

History of past illness 

She had a fracture to her right leg about 10 years ago 

General examination 

The patient was conscious and conscious, coherent and co operative and was oriented to space and time 

- pallor : present 

- icterus : absent 

-cyanosis :absent 

-clubbing : absent 

-lymphadenopathy : absent

-pedal edema : absent 

Vitals 

- BP : 130/70 

- PR : 80bpm 

-RR: 18 

Personal history 

-diet : mixed 

- appetite : no loss of appetite 

-bowel and bladder : normal 

- addictions : not known  

Investigations 

- the patient was sent for 

An ECG , routine blood work and urine sample
