Anemia case

This is an online e log book to discuss our patient identified health data shared after taking his/her guardian signed informed consent. Here we discuss our individual patient problems through a series of inputs from available global online community of experts with a aim to solve those patients clinical problem with collective current best evidence based inputs.

I have been given this case to solve in an attempts to understand the topic of patient clinical data analysis, to develop my competency in reading and comprehending clinical data including history, clinical finding, investigation




A 17 year old girl who is a resident of miryalguda had arrived to the OPD with complaints of generalised weakness since one month , shortness of breath since a week . 

Chief complaints 
She complains of general weakness since a month , shortness of breath on exertion since 7 days , dizziness on standing up and loss of appetite since a month 
History of present illness 
She is studying in an inter college in a hostel in miryalguda since she was 16 , she reported loss of appetite and a feeling of lethargy since this year and said she felt homesick , she did not like the food and used to eat plain rice , she also said that she doesn’t eat fruits and dry fruits ,
She had visited a RMP for dizziness a month ago and was prescribed iron tablets and she reported that her stools turned black after using the tablets , after a routine investigation about 3 days ago it was revealed that her Haemoglobin of 3.5 gm/dl , so she was referred to KIMS yesterday 

History of past illness 
N/k/C/o diabetes , hypertension , tuberculosis 

Personal history 
Occupation : student 
Diet : mixed 
Appetite : loss of appetite 
Bowels : regular 
Micturition : regular 
No known allergies 
She eats chalk pieces since seeing standard and started eating raw rice since she was 11 

Family history 
Not relevant 

Menstrual history 
-menarch : 11 years 
-periods : were irregular and  gets her period once in 3 months since the past year 
-pads changed : 2 pads per day 
-pain is associated with the first 3 days of her cycle
-no clots present 

General examination 
  • patient was conscious, cooperative, coherent and was examined in a well lit room, patient was well nourished and moderately built
  • VITALS
Pulse rate : 86bpm
Respiratory rate : 14
BP: 110/60 mmHg

physical examination 
-pallor : present 
-icterus : not present 
-cyanosis : not present 
-clubbing : not present 
-lymphadenopathy : not present 
-pedal edema : not present
